When calling to make an appointment,  our staff will ask you for the following information.  This will help them schedule the right amount of time for your visit. 

  • Your health insurance (even if work related or the result of an auto accident)
  • The nature of your problem or injury
  • If it is related to a work injury or an auto accident
  • Your name, phone number, date of birth, and zip code
  • Your referring doctor (who sent you to our physician)
  • Your email address

New patients please check in 15 minutes before your scheduled appointment. If you have not pre-registered online plan to check in 30 minutes prior to your appointment.

Cancellation Policy

If you are unable to make your scheduled appointment, please call 24 hours in advance to cancel.  We will be happy to reschedule your appointment to a time that is more convenient for you.

On the day of your appointment, if you are more than 15 minutes late for your appointment, we will reschedule your appointment to another time.

Urgent Problems

For urgent problems, try to call us as early as possible after 9 am.  We will make every effort to work you in quickly, with proper respect for our previously scheduled patients.  Emergencies will, of course, be promptly seen.

Delays

We try our best to stay on schedule, but because of the nature of our practice, doctor is sometimes called away from the office.  We will make every effort to contact you if a delay can be foreseen and arrange to reschedule your appointment.
