Labor & Industries Cases (Workers Compensation)

If you’ve been injured on the job, we are committed to returning you quickly and safely back to work. We have many years experience working with the Washington State Labor and Industries system. We will work closely with adjustors, nurse case managers and employers to properly manage your care.

Making an appointment at Northwest Rehab for a work injury:
In order to schedule an appointment please have ready the following information:

• your claim number
• your date of injury
• your employer
• your claim manager's name and number
• the accepted condition/body part of your work injury

Please be aware that our staff will verify that your claim is open. If we cannot verify coverage or if there is an issue with your claim, you may still receive treatment but you will be required to pay cash.

Motor Vehicle Accident (MVA) Information

Our office has had many years experience in treating injuries as a result of motor vehicle accidents. However, understanding your insurance benefits can be complicated. If you have been involved in an MVA and need treatment please read the following to help understand your benefits and responsibilities.

What you will need to schedule an appointment to be covered under PIP:

• your auto insurance for your car or the vehicle you were in
• your  PIP (personal injury claim information) claim number
• your claim adjuster's name and phone number
• the address where to send medical  bills
  

Please be aware that we will need to verify that your claim is open. If we cannot verify coverage, you may still receive treatment and you will have the option of paying cash or setting up a subrogation agreement with your private health insurance.
