programs. Please consult your insurance carrier
to assure Dr. Brzusek is covered under your plan.
Please be aware that some,
and perhaps all, services provided may be
considered as not being reasonable and necessary
under your medical plan benefits. You are
responsible for payment regardless of any
insurance company's determination of usual and
customary rates or determination of whether or
not services are reasonable or necessary. This
pertains to all insurance and motor vehicle
accidents.
If a workman's compensation
injury is determined to be invalid, the patient
is responsible to provide us with their current
Health insurance information or to pay the balance in full.
